Medical Benefits
The North Central Group provides a medical benefits plan that offers set benefit coverages for the most common everyday health care needs at an affordable price. You have the option of electing health care for yourself, your spouse and/or your dependents.
Dental Benefit Plan
The North Central Group Dental Benefit Plan will provide a complete program to ensure your dental health. Coverage is included for important preventative care and for treatment needed as a result of dental disease or accidental injury. You have the option of electing dental benefits for yourself, your spouse and/or your dependents.
Flexible Spending Account (FSA) for Medical Reimbursement and Dependant Care
This plan allows you to take advantage of Section 125 of the Internal Revenue Code. The Best flex plan increases your take home pay by allowing you to pay certain insurance, medical and dependent care expenses before taxes. The Best flex is not an insurance policy. Refer to the Best flex Program Summary in your benefits enrollment folder. The Health Care FSA has a $6,000 plan year maximum. The Dependent Care FSA has an IRS imposed maximum of $5,000 per year “single head of household” or “married, filing jointly”; $2,500 if you are married and filing separately.
401(k) Saving Plan
The North Central Group 401(k) Savings Plan will enable you to build savings to achieve financial security for you and your family's future. Team members must complete six months of employment, be 21 years of age, and work at least 25 hours per week. The entry dates are the first of each quarter.
Team members may contribute from 1% to 80% of their annual salary. The current employer discretionary match is 50% of the first 5% of contributions that the team member makes. Matches are made twice per year after June 30 and after December 31.
Voluntary Group Life Insurance
You have the option to purchase term life insurance for yourself, your spouse and/or your dependents. You can elect five times your annual salary up to a maximum of $150,000. Your spouse qualifies for 50% of the amount of coverage you elected up to a maximum of $25,000 and dependants qualify for $5,000 or $10,000.
Voluntary Short-term Disability Insurance
Short-term disability is protection that helps replace your income for you and your family if you are unable to work because of a non-work related illness or injury.
Voluntary Accident Insurance
Accident Insurance can help prepare and see your family through the unexpected. This plan provides benefits for initial care and treatment in addition to the follow-up care that you may need.
Voluntary Critical Illness Insurance including Cancer
Critical Illness is designed to supplement your major medical coverage to help with the high costs of treatments for critical illnesses. Benefits may range from $5,000 to $50,000. Illnesses covered include heart attacks, strokes, cancer and major organ transplants.
Vacation Pay
The North Central Group believes that our team members should be given the necessary time for rest and relaxation. Team members are eligible for one week of vacation after only six months of employment, an additional week at one year and two weeks on your second through fifth anniversaries. Three weeks are provided after five years of employment and four weeks after 10 years of employment.

Educational Assistance Program
Our Educational Assistance Program provides an opportunity for team members to gain additional skills and knowledge through coursework from an approved educational institution. Team members who work an average of 20+ hours per week are eligible after six months of employment. Upon receipt of a passing grade, team members will be reimbursed for 100% of books and tuition fees up to $1,000 per year.

This plan is designed to continue your pay if you are unable to work because of a non-work related illness or injury. It provides you with coverage at 60% of your base wage with a maximum of $1,000 per week. The benefit begins on the 15th day following the illness or injury and continues up to a maximum of 90 days (from the date of the illness or injury). These benefits are provided by the employer at no cost to you.
Long-Term Disability Insurance
If you are unable to work because of a non-work related illness or injury for more than 90 days, on the 91st day you are eligible for the benefit. This plan provides you with coverage at 60% of your base wage with a maximum of $10,000 per month. Benefits begin on the 91st day. These benefits are provided by the employer at no cost to you.
Basic Life and AD&D
The North Central Group provides you with a $30,000 Death Benefit and a $30,000 Accidental Death and Dismemberment Benefit. These benefits are provided by the employer at no cost to you.
Voluntary Group Life Insurance
You have the option to purchase term life insurance for yourself, your spouse and /or your dependents. This coverage is available through UNUM Lifestyle Protection. The available coverage levels are:
